LakeSide Forms

All individuals using the LakeSide facility (this includes players practicing with outside teams, participating in clinics, etc.) are required to register by completing the following form and provide to LakeSide management prior to first use:

1. Individual Insurance Form

Each league play team is required to complete the following:

1. Soccer League Team Registration Form

2. Coach Registration Form (Required for each team coach)

Each individual (league team member) is required to complete the following:

1. Individual Registration Form

2. Individual Insurance Form

Anyone who is a certified and wishes to referee LakeSide league games:
